What Is Concierge Medicine? A Guide for Austin Patients
By Ann Czarnik, M.D., Board-Certified Emergency Medicine Physician and Co-Founder, Amanecia Health
If you’ve heard the term concierge medicine but aren’t exactly sure what it means, you’re not alone. In Austin, the phrase is used to describe everything from traditional membership practices to telehealth apps that promise direct access to a doctor.
Concierge medicine refers to a model where physicians maintain smaller patient panels so they can offer more time, faster access, and a more direct relationship with their patients.
Concierge Medicine Defined
Concierge medicine is a healthcare model where patients pay their physician directly for care, either through a membership fee or per-visit pricing. Because physicians are less dependent on insurance reimbursement structures, they are able to see fewer patients, offer longer visits, and provide more direct access to care.
I’ve practiced medicine in Austin for nearly 17 years, most of that time in emergency medicine, where I saw firsthand what happens when patients don’t have a physician who knows them, who can get them to a specialist quickly, or who’s available when something comes up at 8 PM on a weeknight. That’s why I co-founded Amanecia Health, to build the kind of practice I’d want for my own family.
Here’s what concierge medicine actually is, how it works in Austin, and how to figure out whether it’s right for you.
What Concierge Medicine Actually Is
Concierge medicine is a model where patients pay their physician directly, either through a membership fee or per-visit pricing, in exchange for a level of access and attention that traditional primary care often cannot provide. The trade-off is straightforward: your doctor sees fewer patients, which means more time with each patient, same-day or next-day availability, and direct communication when needed.
What it’s not: a luxury product with no substance behind it. The physicians in a well-run concierge practice are doing the same clinical work as any doctor (diagnosing, treating, prescribing, referring, coordinating care) but with more time, fewer barriers, and a real relationship with the patient.
At Amanecia, we don’t charge a monthly membership fee. Our model is pay-as-you-go: you pay for visits and services as you use them, and we accept HSA and FSA. This makes concierge-level care accessible without the $5,000 to $15,000 annual retainers that some practices charge.
How It’s Different from Traditional Primary Care
If you’ve been in Austin’s healthcare system, you already know the pain points. Three-week waits for an appointment. Fifteen minutes with your doctor, most of it spent re-explaining your history because the last visit was six months ago. Referrals that take weeks to process. No way to reach your doctor after hours without going through a call center or showing up at an ER.
Concierge medicine solves these problems structurally, not by working harder within a broken system, but by building a different one. Fewer patients per physician means longer visits, faster access, and continuity. When I see a patient, I already know their history, their medications, their goals, and what we discussed last time. That saves time and produces better outcomes.
Typical concierge medicine benefits include:
- Same-day or next-day appointments
- Longer visits with your physician
- Direct messaging or phone access to your doctor
- Coordinated specialist referrals
- More personalized care plans
The other difference is coordination. In traditional care, you’re often managing your own referrals, chasing lab results, and navigating specialists on your own. At Amanecia, we handle that. We get patients to specialists in days, not months. We follow up on labs proactively. We coordinate across providers so nothing falls through the cracks.
The Concierge Medicine Landscape in Austin
Austin has a growing number of concierge and direct primary care options. Some are traditional membership practices where you pay an annual fee for a dedicated primary care physician. Others are tech-forward startups that offer app-based access. And then there are practices like ours that combine the clinical depth of emergency medicine with the accessibility of the concierge model.
Here are some questions worth asking when evaluating your options:
What are your physician’s credentials?
There’s a wide range. Some concierge practices are led by board-certified physicians with decades of experience. Others are staffed primarily by nurse practitioners or physician assistants with limited physician oversight. Both can provide good care, but you should know who’s leading your care plan.
What’s the fee structure?
Annual retainers in Austin can range from $2,000 to $15,000 or more. Some practices charge per visit instead. Some accept insurance for covered services and charge separately for the concierge access. Make sure you understand what you’re paying for and what’s included.
How do they handle urgent needs?
This is where concierge medicine should shine. Can you reach your doctor directly (by text, call, or video) when something comes up on a weekend or evening? Or are you still routing through a call center? At Amanecia, patients reach me or Dr. Demchur directly.
Do they offer in-home visits?
Not all concierge practices do. Amanecia was built around in-home care and telemedicine. We come to you, whether that’s Westlake, Tarrytown, Barton Creek, Bee Cave, East Austin, Mueller, or downtown. No waiting rooms, no driving across town.
What services do they actually provide?
Some concierge practices are strictly primary care. Others, like Amanecia, offer a broader range: weight loss medications, hormone replacement therapy, peptide therapy, urgent care, wellness, and longevity services. If you’re going to invest in a physician relationship, it helps if that physician can manage more than just your annual physical.
Who Is Concierge Medicine For?
The short answer: anyone who values their time, wants a real relationship with their doctor, and is frustrated with the traditional healthcare experience. But in Austin specifically, we see a few common profiles:
Busy professionals and executives
Who can’t afford to wait three weeks for an appointment or spend half a day at a clinic. They need a physician who’s accessible and efficient.
Families with young children
Who want a doctor they can call when their kid has a fever at 9 PM instead of debating whether it’s worth an ER visit.
People managing ongoing health goals
Like weight loss, hormone optimization, or longevity, who want a physician actively involved in their progress, not just writing prescriptions.
New Austin residents
Who moved here and can’t get into a good primary care practice for months. Concierge medicine gives them immediate access to a physician while they navigate the Austin healthcare landscape.
Patients who’ve had bad experiences
With rushed, impersonal care and want something fundamentally different.
How Amanecia Health Works in Austin
We’re not a traditional concierge practice and we’re not a telehealth app. We’re a physician-led practice built around two things: clinical depth and accessibility.
Every patient at Amanecia works directly with a board-certified Emergency Medicine physician. In Austin, that’s me and Dr. Nora Demchur. We also have David Lala, PA-C, and Kainoa Warnock, RN, on our clinical team. We deliver care via telemedicine and in-home visits across the greater Austin area.
There’s no membership fee and no long-term contract. You pay for the care you receive, and we accept HSA and FSA. Most patients come in for one thing (weight loss, an urgent care need, a hormone question) and stay because having a physician who actually knows you turns out to be something they didn’t realize they were missing.
Our services in Austin include medical weight loss with GLP-1 medications, in-home urgent care, telemedicine visits, hormone replacement therapy, and wellness and longevity treatments including peptide therapy, NAD+, and B-12. If you need a specialist, we coordinate the referral and follow up to make sure it happens.
Concierge medicine is ultimately about having a physician who knows you, understands your health history, and is available when care is needed.
If you live in Austin and are exploring concierge medicine, we would be happy to talk through how the model works and whether it is the right fit for your healthcare needs.
